NDIS Connection Desk - Lalor Park Community Centre

One-on-one, 30-minute assistance to access the NDIS:

• Assisting in filling in forms and information on what is needed in the medical reports and supporting evidence.
• Assisting existing NDIS participants to utilise their plans and make the most out of it.
• Referrals to mainstream and community supports.
• Connecting to services needed.
• General information around any NDIS matter

30min sessions available on the below dates between 11am-2.30pm:

• Wednesday 21 February
• Wednesday 20 March
• Wednesday 24 April
• Wednesday 22 May
